Does she shave her butt cheeks? Could be an ingrown hair? Not sure.. That's the only thing I could think of.

I have the same issue - it's usually caused by the same thing as "backne" or back acne - and that is sweating in your clothes and keeping those clothes on (or in this case underwear) overnight or even longer... So the solution is to change your underwear often and also wash your bum with a body wash made for body acne - and if she has current blemishes she can use some benzoyl peroxide on them to help them go away - but use some gentle with only 2% BP - I googled for a solution to the problem and it basically says its just clogged pores is all - not really acne - so another way to help unclog the pores is to sit on a hot towel to bring everything out of the pores then use like a witch hazel toner to reduce swelling and clean out the pores (or you can mix done with water and just wash your body with it) and then spot treat with the BP - hope that helps!

I would go to the doctor too if I was her just in case it is something more than acne, like an STD. I'm not saying this to be offensive or to scare you/her but it's better to be safe than sorry, just in case. Either way, the doctor should be able to help treat it whether it's clogged pores or not.

I'd go check with the doctor. she shouldn't be ashamed of speaking to a doctor about that, I'm sure they see some crazy things & red spots on a bum wouldn't make that list :)

For the hot towel, use like a face towel and run it under hot hot water and ring it out and then drape it over the edge of the tub or toilet seat and sit, or after its wet with hot water throw it in the dryer on high heat for a bit so it's still wet but hot.

What material is her undergarments? If they are polyester this will cause break outs in the butt. Try to wear cotton underwear and see if that helps. And butt acne is not uncommon. If it really is an issue see a doc. But definitely tell her to check the material of her undergarments. Spandex or polyester is not good for that area. If you sweat it clogs the pores n causes acne. If you wear cotton instead, it absorbs some of the sweat.

I would suggest switching to cotton undies. It actually helped me when I had breakouts right on top of my bootayyy aha.

Honestly the best advise I could give is to see a physician! Nobody knows her skin in order to give any medical advise. One of the biggest things we learn as Estheticians is to never give out any sort of skin medical advise because you never know what is going on within her body. I completely understand being embarrassed but it's better to get professional advice than to experiment and make it worse. Nobody will judge her!! And it kinda worries me that it's been happening for years. I'm really not trying to come across as mean at all! I just want to make sure she gets it completely resolved and understands what the issue really is. I wish her the best :)

Yeah how do you get any towel hot? With hot water... Sorry I thought that was a simple thing to do... But I personally get pimples on my butt so I'm only giving suggestions that have worked for me - I don't have an STD or anything like that... They look n feel like pimples and they go away after I use the methods that I suggested... Doesn't mean it will definitely work for her - but it worked for me so figured I'd give suggestions.

I second Diana, my guess it's some form of acne. I occasionally get a random red blemish or pimple on my butt but it doesn't bother me. I say treat it as acne but if it is really bad as in bad face acne she should go to a doctor to treat it effectively.

Yeah and when my acne goes away it leaves acne scars behind.., which are flat and look brownish like acne scars...

I'm only responding to the info I've been given and the OP said it was acne/acne scars on the bum.

Flat & reddish sounds like a rash to me. Try hydrocortisone cream. Professional medical advice can be sought after for anything related to the human body, both serious and not serious. If this has been occurring for years, she should see a doctor.

Try using a body wash made for sensitive skin that has no fragrance or something that only has natural fragrance. Also try wearing only cotton underwear as synthetic fabrics can trap sweat which can cause irritation. It's probably nothing serious so don't stress!
