I have been seeing "face mapping" where depending on where a specific breakout is is will tell you what the issue is like if you break out on your forehead it's usually because of a bad diet or near your ear because your dehydrated or whatever it may be. Anyhoo I'm just wondering if it has some truth to it or if its just a total hoax.

I believe it does! I've seen articles on that saying it's from an asian background and I take believe it for the most part. like for example smokers, you can see the effects in their mouths so in a way its your body telling you something is wrong. and coming from an asian family I've heard it all my life 😂

That infographic is misleading. The things that it mentions (smoking, dehydration, poor diet) effect the entire body, of course you will see an effect on your face. No particular "zone" of the face is linked to any organ system. How would they be linked? There is no direct nerve or vein or anything else connecting your eyebrow to your liver.
Smoking causes issues in every bodily system. It isn't the lung damage that makes a smoker get wrinkles or zits on their face. It is the exposure to toxic chemicals from the cigarette. The wrinkles around the lips that you often see on smokers are from pursing their lips to inhale (coincidentally, you see the same wrinkles in people who speak a language with lots of sounds that require the same shape, French for example).
If you are dehydrated, your whole body is affected. Your skin will become less taut, you will get headaches, your digestive system slows, and your bladder and kidneys can shut down (if it is severe). Dehydration also causes confusion, sluggishness, and slower healing from abrasions and contusions (including acne).
Poor nutrition also affects the body. Deficiencies in different vitamins/minerals/amino acids can result in a multitude of symptoms, from fatigue to jaundice to acne and worse.
While the infographic is informative about different effects that certain bodily ailments can have on skin, it's basic premise (that certain organs directly affect regions on the face) is false.
