It seperates on my cheeks and nose all the time, and I just applied it. I moisturize, and I've tried using a ton of different primers yet it still happens no matter what foundation I use.. Please help it makes me so frustrated!

Are you waiting to give both moisturizer and primer time to absorb before applying them? Time before foundation? Also, have you tried using your fingers to apply? Light exfoliation?

Do you have oily skin, perhaps? what foundation are you using?

What Rozberry said. moisturize (wait 3-5mins), prime (wait 3-5mins), apply foundation (wait another 3-5mins), & then apply concealer or powder. that's what caused my foundation to separate or look kind of cakey.

What are using to apply it? Setting it with loose powder helps to "dry down" the foundation so it is no longer wet and therefor creasable or anything like that.
