But don't they trail along the ground, so there'd be dirt in it? Or do they farm snails which are in a clinical environment?

This is really popular in Korea and don't worry they don't get the gel thingies from the ground. They extract it in some way I do not want to imagine haha. Its supposed to be very moisturizing and anti aging. My friend who went to Korea for a month tried this and she got rashes. I'm sure many people love it though as its popularity is growing as well as Snake venoms. Yup, weird stuff but helps collagen production and repairing moisture. :)

I think it is the slime excreted by the snail.
