My nails are REALLY REALLY weak and whenever they grow out I end up breaking them because they are so fragile. I really need tips cause I want strong and healthy nails :)

Sounds like you need a good nail routine. Here is mine:
I remove my polish with a non acetone remover. Acetone is horrible for the nails.
next I lightly buff around my cuticles to remove ridges
I then push back my cuticles
I now reshape my nails if needed with a file
next I apply two coats of strengthning polish.
After that has completely dried, I apply a cuticle oil. just keep your nails protected with the strengther polish ans keep your cuticles moisturized and your nails should start to get better

Use top coat, and then a color. And two coats or something..
