Bad Skin -Please Help!

So I'm 15, I've always had fairly good skin, but recently it's been AWFUL. I've had blackheads for as long as I can remember, but only at the end of my nose, where most people have them. But in the past few months I've noticed them spread to the creases of my nose (where it touches my face) the apples of my cheeks, my chin, my upper lip and my forehead!
My skin is fairly normal. In the winter it gets a bit drier and in the Summer it gets a bit oily. Actually I've noticed that my blackheads are in the areas where my face gets slightly oily in the Summer.
I have a fairly good diet, the only bad thing I eat is chocolate. I drink nothing but water, I usually go through 2 500ml bottles a day. I hardly ever get spots so I don't suffer from acne and never have.
My skincare routine is: In the morning, I wash my face with Angels on Bare Skin from Lush, then I rinse with cold water to close pores. I apply eye cream, and then Cocoa Butter moisturiser. In the evenings I take my makeup off with a cleanser and apply moisturiser. I use a face mask once a week, and recently I've been trying to exfoliate twice a week. I also have makeup free days at least once or twice in the week, to give my skin a break.
I don't know what I'm doing wrong, I really need help!!!

cocoa butter clogs pores, I would stop using it. Tons of water is great. It's actually impossible to close pores, although you can tighten then with cold water. I suggest trying cetaphil moisturizer instead, olay cleansing brush or clarisonic brush, Control Corrective Salicylic wash or Neostrata Anti-bacterial face wash. Never touch your face or lean on it, n picking, change your pillow case every night, clean cell phone or other mobiles with alcohol..

get some boiled water,
lemon juice&
salt

mix it all together then dip a clean cloth and pat on the areas needed
i suffer from blackheads but this defiantly helps and it doesn't cost much!

This will save your life i know it did for me EVERY TIME to make it work even faster for the first week mix a dab into your moisturizer also stay away from scrubs it just moves acne around to other areas of the face an was you billow cases constantly. keep your hair up an out your face for 3 weeks :)  

I'd suggest going to see your doctor or a dermatologist and see what they can suggest. It may just be hormones as you are a teenager.

Wow, you are really meticulous with your skin! Well, I can't tell you how amazing Murad is! It saved my skin! It is seriously the best thing that ever happened to my skin!!! ^.^

Hey :) I'm 16 so I have a similar price range to you =P honestly the best thing you can get are pore strips , but even then if you don't wanna cash out for that , apply hot water on your face n apply a thick layer of PVA glue on the affected areas ! :) pulls the buggers out haha =P hope this helps :) also , if you have spots , before you go to bed , add a blob of sudocrem or any antiseptic cream on the spot, n in the morning they would if shrunk :) its done me wonders ^_^

I've found that blackheads are actually the easiest kind of Acne to get rid of. It's oil and dead skin cells that become trapped in the hair follicle, unlike the big cystic ones that form from within. The dark color appears when a chemical reaction occurs between these things and the melanin (something else your body produces naturally). It's not dirt trapped in there like I used to think!

I've found the best way to get rid of my blackheads is to first steam my skin (to heat up the oils) and then use a soft facial brush and face wash to deep clean my pores. The steam is really important because it opens everything up and allow the debris to be cleaned out. I've also had some luck with the Biore strips, which pull the oil and dead cells right out of the pore!

have U used something new lately? if so pay attention to the ingredients, there might be something...
as for the black heads i would say go for the St. Ives apricot scrub
