I'm not really the best at smokey eyes, but online I found a few charts that may help you out

A smokey eye involves a darker color on the lid & your lid, crease, transition color should be blended so well that you cannot see where one color starts and the other ends. my phones dying right now I'll see if I can find a good picture to best describe how to apply in a few minutes

This is the gist of a smokey eye, however I feel it should be smoked out a little more. Adding to what Sarah said, the colours blend up, not across. A traditional smokey eye does not have eyeliner.
I believe what Kaitlin posted is a form of cut-crease.

The only real issue with Hannah's picture is the fact that there is a more evident transition horizontally than vertically (which is the direction the transition should go ). Basically start at your lashline with black and get lighter and softer as you go up diffusing the color so that it looks seamlessly blended and smooth.
