Breaking Out Bad!

Breaking out really bad, I use cetaphil gentle face wash but I try not to use facial moisturizers considering they always break me out but I've been using jergens lotion on my face & I'm breaking out too! help?!

I would break out a lot also but then I started to use cerave cleanser &the moisturizer
