I think the only real solution is to go to dermatologist. It is a little bit painful but your face will be clean. :-)

What I do are pore strips (they don't work for everyone, but they work for me). It works best if I do them after my showers so my pores are open from the steam. I also have a Clarisonic, which is designed to send sonic pulses into your skin and loosen the blackheads. A salicylic acid cleanser will also work to dissolve the impurities stuck in your pores layer by layer. A monthly trip to get a facial will also help a lot, but is not a necessity.

Cleanse (and remove with damp.cotton/damp sponges twice), tone, exfoliate, steam, manual extrations (wear latex gloves and wrap fingers with tissue, stretch skin, push skin together and wiggle to remove blackheads) apply disinfectant to area once done, pore strips (from store or DIY using 1 tablespoon milk mixed with 1 table spoon unflavoured gelatine and put in microwave for 10sec, apply with paintbrush and leave for 10min), tone area pore strip was removed from (nose/chin), massage to loosen blackheads, tone to remove massage oil/cream, mask to draw out impurities (clay mask, or DIY using egg whites, remove after 15-20min with warm water), tone, moisturise.
