The polish I always reach for when I need white is ULTA's Salon Formula Nail Lacquer in Snow White. It's super cheap but very opaque and last about a week.

I've found Sally Hansen Nailgrowth Miracle in 120 White Tip to be great! It has a nice large, flat, dense brush which I think is a bonus because it only takes two swipes (one on my pinky haha) to cover the entire nail :-)

I always use Alpine Snow... I really like it but sometimes I do have a few problems with application because of the formula. Could just be my bottle, though. As long as I'm careful I can get it perfectly opaque in 2 coats!

Pure Ice- Superstar. You can get it at Wal Mart for 1.99, and it's opaque in two coats.

I used to have Sally Hansen's White On and I loved it but then it thickened up and got REALLY goopy and hard to apply. So I don't really like that one, I am trying out Wet N Wild's White Creme right now but its more of an off white rather than a pure white.

White On White by China Glaze!
I only needed one coat and it was opaque and I couldn't see any of my nail through the nail polish.
