Use a primer and a setting spray I found that makes my skin look a lot better I use nip+fab sheer makeup fix for my primer and urban decay de-slick as a setting spray. But you can change that according to your skin type

Maybe your using the wrong foundation or not applying correctly..What's your skin type? And use a good primer before applying foundation and give it some time to dry! and set your makeup with a translucent powder or setting spray!
