Almost everytime I apply concealer, it looks caked. I apply a little bit and blend it out as well as I can, I also moisturize before! (and it's with basically any concealer) any tips on how to blend better?

Maybe put primer before. Or maybe its the concealer that you are using.

It could be that you're not a primer, however I've just recently started using primer & I've never had this issue.
it could be the way you're applying it? for example, if you're putting concealer under your eyes, don't just apply it right underneath the eye, blend down into your cheeks. also, only use a little. I wish I had kept the photo that better showed what I mean >.< lol. also, maybe try buying a concealer that's a shade lighter than your skin tone. that way it will look like it's highlighting your skin instead of just.. being there I guess xD this might help it look less cakey on the skin.
also, using your fingers to blend warms up the product & makes it easier to blend into your skin.

Maybe you have dry skin and need to try a stronger moisturizer?
