Both after foundation. If you're concealing large areas it's ok to do it before foundation, but it doesn't make sense to conceal a pimple or whatever and then blend your foundation. It's going to move the concealer all over the place. Same with contouring. You don't want to move the shadows you've created by buffing your foundation. Not to mention that after you put on foundation, you may not actually feel the need to conceal some things that you would have concealed with your bare face, because they won't be as visible.
This is also why I am not a fan of using extremely light concealer. Concealer should basically match your skin tone so that you can dab it on top and just blend to edges to make it look seamless, but not discolored by having this one little bright patch. The one I use is an almost exact match after I've put on my foundation and it works like a charm.
