Do you usually wear a shade that matches your skin tone or do you use a lighter shade? If you can recommend a good concealer for medium/tan skin and the shade I would be very grateful :)

I use Benefits bo-ing, and they have 3 range of different colours, I have the lightest one, it's really good coverage and a lot of my friends that have tried mine out have ended up buying it themselves. Wearing it underneath your foundation on your problem areas is the best way to wear it I find, however it is good to use on top of makeup on problem areas. I also use it as a highlights sometimes :)
Hope this helps

I'm fair, so usually the lightest shade in most brands are almost my skin tone.

Ok thank you! I wasn't sure because I've seen some tan girls use a lighter shade of undereye concealer than their skin color

Go lighter but not too much, it should match the lightest shade in your natural skin tone so it looks bright but natural at the same time (: also, make sure you blend thoroughly! you don't want to look like you've got a chunk of one color under your eyes!

I've always thought it was the norm to go one shade lighter in terms of concealer.

I use a purple rosy color because my under eye circles are a bit yellow ;s
Depending on your under eye area color you should use yellow for blue or purple, green for red or full coverage in your skin shade if they are just dark :D
