There's a distinct difference between artificiality and decoration. I consider a winged eyeliner or smokey shadow to be decorative. HUGE false lashes that don't at all blend into your natural lash and don't even pretend to are artifice - i.e. "fake".
It's like that old Clairol tagline: "Does she or doesn't she?" If you're not sure if those are just thick, gorgeous lashes with a coat of mascara or glued on, you're doing it right. Point being that it's not about the drama of the look...it's about the execution.

I would never say somebody looks fake based on there makeup choices because makeup is art but if you google 'human barbie' and all the plastic surgery crazy women thats my idea of fake

The only time I call someone fake is when they go way over board with the make up. Like their eye brows are clearly drawn on and way too much eyeshadow and I guess when the makeup doesn't fit with the scene, like when someone piles all this makeup on to just hangout at home or for everyday... If that makes sense lol

I think being fake is when someone is insincere or trying to be something that they are not (more like not being true to themselves). Maybe heavy makeup or hair dye is totally a part of a person or displays their personality, that's awesome! But when you do things for the wrong reasons and expect certain outcomes is where insincerity and not being true to yourself occurs.

To me there are two kinds of Fake, a personality fake (like Kim kardashian or a person who is not sincere) and a fake when it comes to the body like obvi Plastic Surgery. I don't consider anything that comes to makeup as fake. Because I wear makeup and I am not close to fake in the least bit.
