How do you know which one to wear? is there even a difference?

Most bb cream offers a lighter coverage, only there to even out your skin tone. it's lighter than a foundation and looks more natural. a foundation on the other hand has a thicker feel to it, and has a better coverage. also, you could have put this and your drugstore question together, so other peoples' questions won't get bumped down

A bb cream is sheer coverage most of the time. As Shreya said, it evens out your skin tone. I personally prefer bb creams because it feels like your skin and it's not thick so it's a lot more comfortable to wear in my opinions. Foundations on the other hand vary in coverage. Usually medium-full coverage (at least in my experience) They are thicker and I personally can't stand wearing it.

BB cream is just like a correcting lotion, it's very light and just smooths out the coloring of your skin. I've never personally been a fan of BB creams. Foundation is a higher coverage more concentrated color, it's is more of a liquid and less lotion like. I like foundations blended out with a beauty blender so they don't feel so heavy but you still get a nice amount of coverage.

If you want great coverage I would use a good foundation, but if your just want something to smoothing your skin and give a flawless, yet natural look use a BB cream its light and doesn't feel cakey on skin ever

I honestly prefer foundations because they tend to be much more longlasting. I also think you should have a separate skincare routine, and I also usually don't trust makeup brands to do my skincare. You can buy a light coverage foundation to have the same bb cream effect, but foundations overall are a bit pricier, but worrh it in my opinion.
