I use a shade a few shades darker than my skin as my transition color and use a fluffy brush to blend using windshield wiper motions right where two colors meet to blend them together (:

Always use a clean brush. it may be a better investment to get a color switch pad than to get a bunch more brushes. hold the brush at the tail end so that you have less control. barely touch the brush to your skin. you don't need to push or rub to blend everything out, you want to lightly diffuse things. when you're blending, use tiny tiny movements. don't sweep the brush in big circles or things will get muddy quick. blending is usually done with either small circles or short windsheld wiper motions.
windshield motions will move the color and help to blend it out and small circle movements help to blend two colors together.
a big fluffy brush is good for blending your transition color but to blend two colors together on the lid or to blend at the crease a small fluffy brush that allows you to control movement better will be a better option.
as far as colors for transition shades, you can use one in the same shade/color family that is lighter than your crease to blend it and create more of a diffused look. I see a lot of YouTube tutorials that double a base shade (an eyeshadow very close to your skin tone that you put from lash to brow bone) as a transition shade but I find that when I am working with brighter colors I need something else to make them really "flow". so for a look that I have a deep wine in the crease, I'll use a reddish pink to transition. on a more natural look where I use a red-brown in the crease I'll use an orange-brown to transition. for examples.
