I began by using Urban Decay eyeshadow primer as a base.

Broken (Urban Decay, under brow, LE, Theodora Palette)
Brows - Anastasia Beverly Hill Brow Wiz in Soft Brown.
Lashes - House of lashes Noir Fairy
Lips - Morange Lipstick (Mac Cosmetics) and Harlot Lip Tar (Obsessive Compulsive Cosmetics)
