If it's an infection it is going to need cleaned out. Go to the doctors and they will prescribe medicine most likely an antibiotic if it's a bacterial infection. It doesn't look infected in my opinion. It looks like you probably scratched it with a nail

I had an infected tattoo and it was the most painful thing I've experienced so far. I would really recommend that people make sure to follow their artists instructions. I didn't go to a reputable artist this time and instead had to follow directions online, http://aquaphorfortattoos.com/how-to-treat-an-infected-tattoo/ helped a bit but no substitute for real advice.

You can probably use the same advice given above for most infections as long as it is a skin infection, rather than internal.
