What causes white heads & what gets rid of them?

White heads are my biggest skin issue. Dozens around my chin.

What's your skin care routine and your foundation routine?

I am bad with sticking to a full skin care routine. Right now it's like, remove my makeup with a makeup remover, wash my face; I'll use one of those sonic brushes every second day with a face wash. Toner and moisturizer, sometimes a little dab of tea tree oil on some bad spots. and a few times a week I use the a GlamGlow SuperMud.
Foundation is MUF Primer, Revlon Color Stay foundation, MUF HD Setting Powder.
I've always been plagued with white heads, seriously dozens mostly focused on my chub and jawline, but I've never known what causes them and what gets rid of them.
