How often do you exfoliate?

Say NO to Proactiv! Since you said you don't have very many breakouts, I think you should dial it back and use gentler products. Proactiv, along with most acne and skin-clearing products, is very strong and really irritating, actually. Try something simpler. Don't exfoliate too often, that is also irritating and the scrubbing beads can block pores! For a drugstore face wash option, try Cetaphil, Purpose, or an Aveeno brand cleanser. These are some of the most common over the counter cleansers recommended by dermatologists.

You should exfoliate more. It sounds more like you have white heads trapped under your skin. Try an exfoliating scrub with salicylic Acid in it.

Exfoliating is key...so you got some great advice already. But you don't want to overdo it either, look for "round exfoliators" and be kind to your skin. I have two gentle exfoliators that I use. One by Philosophy (my favorite too) called Micro-Delivery Exfoliating Face Wash and the other by Bliss, the Fabulous Foaming Face Wash. Then I follow up with Salicylic Acid (right now I'm using one by Clean and Clear). A Glycolic mask is good to use too (like by Serious Skin Care). I keep my skin care routine pretty simple and over the weekend I went out for a chemical peel and the esthetician was pretty impressed with my skin.

Another very very good drug store cleanser, that isn't known about, but should be up there is CeraVe. The ingredients in CeraVe are found in more expensive higher end department store products, ingredients that no other drug store product has. So for it to do the same thing higher end products do, for 1/5 the cost... it's perfect!
By the way, I've been using since the beginning of this year, and I've suffered from horrible acne before that. Check out a beauty's blog (Soma S.) about the product: http://www.kabukivoodoo.com/2011/07/skin-care-cerave-must-have-staple-in-my.html
Changing my skin care regimine to CeraVe; in addition, to my acne fighting topicals, my face has cleared tremendously! I don't have my horrible cystic acne breakouts anymore, and I don't get too much pesty little pimples either.
I also did start doing a scrub and mask 2x a week. This has helped fade my acne scars, and take off that top layer of dead skin that can clog my pores. I just use any Appricot (sp?) scrub, and St. Helen Mint Julep mask.

you probably only need a salicylic acid spot treatment for those. i don't know if i would use it in conjunction with proactive though. i have never used proactive so i'm unsure of what's really in it.
