Primer or a white eyeliner pencil like NYX Milk pencil. It needs something to stick to. Don't put too much on the brush at once. And you'll still have fall out. Makeup wipes are the best for cleaning up fall out!
I usually do my eye makeup first before foundation so it's easier to clean up the fall out. I agree with Ruby on primer!! You could also get the brush damp to grip the shadow and tap it to your eyelid.
Make sure to keep your brushes clean and free of old makeup for your next use. A lot of people skip this step and it makes a huge difference. When applying you can use an eye card or a CLEAN business card and place it in the crease of your lashes while you apply, creating a barrier between where you are applying your makeup, and where you don't want it to fall. If you apply your loose shadows while your foundation/concealer/moisturizer is still drying, it will hold it much much better throughout the day or night. Once you have your eye shadow on an secure, put on your mascara and final/only coat of eye liner. (I like to use a compacted brown powder as my liner). (:

Get pixie epoxy from fyrinnae! Use it over the primer and wait for the epoxy to get sticky before applying the shadow. It will stick, stay and look fabulous!
What kind of brush do you use? I prefer to use a flat shadow brush (The Urban Decay one) To apply, I gently pat the shadows on, really pressing them into the base. Sticky bases and primers help of course but I find with loose shadows there is almost always going to be a little fall out. And about it getting on the lashes, usually mascara will cover it up, but if its really bad I'll run a qtip with makeup remover over my lashes to clean it up:)

You could get something like a mixing medium so you can mix that and the loose shadows together so it won't be as messy.
