What does acne around the cheeks mean?

Every so often I get almost cystic acne on my cheeks and I don't know what's causing it. I'm washing and treating regularly.

It can mean a lot of things like change in diet, if you don't wash your sheets regularly, if you touch your face to much or your phone or just hormones.
